Six Hawaii Island community care services case managers were among 24 others recently honored for providing exemplary care by ‘Ohana Health Plan during its recent CCS provider appreciation and case manager awards luncheon at the Hawaii Okinawa Center on Oahu.
Six Hawaii Island community care services case managers were among 24 others recently honored for providing exemplary care by ‘Ohana Health Plan during its recent CCS provider appreciation and case manager awards luncheon at the Hawaii Okinawa Center on Oahu.
The Big Island honorees who were nominated by their case management agencies are:
• Ian Arruda and Dennis Brody, Care Hawaii
• Elizabeth Carroll, West Hawaii Community Mental Health Center
• Fa‘ata‘ape “Ape” Lavatai, Hope Inc.
• Kristy Lungo and Tara Tina, Mental Health Kokua
Through the state’s Department of Human Services’ QUEST Integration program, CCS assigns these case managers to facilitate the delivery of behavioral and medical health services to approximately 5,200 Medicaid-eligible adults who have severe mental illness and are enrolled in the initiative. ‘Ohana Health Plan has been the sole provider of CCS services since 2013.
CCS case managers meet face-to-face with members to assess their health, plan their care program, and monitor their progress towards reaching their behavioral health goals.
To help ‘Ohana Health Plan members receive the right care, in the best setting, case managers often meet members at the locations most convenient for them including parks and beaches.
